How to Get Started
To request a new patient appointment, you will need to complete the intake process and provide your current demographic and insurance information. Once our AdvancedMD intake system is fully connected, you will be able to submit this information securely online.
What You’ll Need
- Current contact and demographic information
- A copy of your driver’s license or other photo ID
- Front and back images of your primary insurance card
- Front and back images of any secondary insurance card, if applicable
- A current medication list
- Relevant medical or psychiatric history

Before your first appointment, please make sure all requested forms and documents have been submitted. This helps avoid delays and allows time to review your information before the visit.
What to Expect
After your intake information has been reviewed, you will be contacted regarding the next steps for scheduling. Your first appointment will focus on reviewing your history, current concerns, treatment goals, and whether Synergy Family Psychiatric Services is the right fit for your needs.
